Investigation Summary
At 11:12 a.m. on June 3, 2021, Employee #1 was covering the filler and capper position on Line 11. The employee noticed a bottle on its side on the filler section, opened the interlocked gate to stop the machine, and removed the bottle. After closing the gate, the employee proceeded to the capper side where he opened the interlocked gate and swiped accumulated caps from the cap chute with his right hand. As the capper moved up to place a cap on a bottle, the employee's right hand was caught, resulting in a finger amputation. The employee was treated without hospitalization.
