Investigation Summary
An employee was working as a licensed practical nurse in a physician's office. She had received a negative result on a COVID-19 rapid test, but two days later, she received a positive result from a PCR test. The following day, she became short of breath. She went to the hospital. She was admitted for treatment of her shortness of breath and dehydration, and she underwent surgery for an infection she was found to have in one of her wrists or hands. She died three days later, at 4:44 p.m., on September 13, 2021, due to COVID-19. She had contracted COVID-19 from an unknown source.
