Investigation Summary
An employee was working as a dietary aid, at an assisted living facility. The employee was away from work due to reported symptoms consistent with COVID-19 infection, and was tested the following day, but was understood to have become seriously ill before reporting a result. The employer was informed by the coworkers that the employee was put on a ventilator and died from COVID-19 at approximately 5:00 p.m. on September 3, 2021.
