Investigation Summary

At 10:00 p.m. on September 2, 2021, an employee was placing caps and sleeves on propane bottles. The employee reported not feeling well to his supervisor. The employee was taken into the air-conditioned office and spoke with a third party nurse over the phone. The employee was hydrating and continued to sit in the air conditioning. The employee did not want to leave work and was allowed to continue working in the office instead of working in the plant. The employee finished his shift and went home. The wife of the employee called the OSHA office and spoke with the AAD. She said after the employee came home, they went to a pizza place to eat dinner. The employee went to the restroom, and while in the restroom the employee had a heart attack, and was pronounced dead at the restaurant.
