Investigation Summary

Investigation Nr: 138829.015
Event: 07/25/2021
Employee with COVID-19 infection dies

An employee was working as a principal lead hardware board design engineer for a manufacturer of telecommunications equipment when he traveled to the company headquarters for mandatory training in preparation for the launch of a new product line. The employee attended the training sessions for two days until he became ill and called his supervisor from the hotel early in the morning of the third day and reported that he was sick with a runny nose, sore throat, and symptoms of a bad cold. The employee stated that he did not think it was the flu, much less COVID-19, because he was vaccinated. The employee did not report to the training on the last day and flew back to the airport closest to his home on July 16, 2021, arriving after midnight on July 17, 2021. The employee stayed at a hotel close to the airport and then drove home later in the day. After arriving home, the employee was transported by his daughter to the emergency room at a local medical center, where he was admitted, tested positive for the COVID-19 delta variant on July 18, 2021. The employee was placed on a ventilator on July 20, 2021 and died on July 25, 2021 due to complications of COVID-19.
