Investigation Summary
At 8:00 a.m. on August 13, 2021, an employee who had been was providing care and services to clients at a vocational rehabilitation facility was reported hospitalized with COVID-19. A client had tested positive for COVID-19 at some point, but was asymptomatic when in contact with the employee. The employee was placed off work and later hospitalized due to COVID-19. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
