Investigation Summary
On November 16, 2020, an employee who had been working for a manufacturer of specialty industrial-grade striking, digging and cutting tools tested positive for COVID-19 infection. The employee's last day of work was Friday, November 13, 2020. The employee was hospitalized multiple times, but the employer does not know if the hospitalizations were COVID-19 related. The employee had underlying medical issues. On Saturday, April 24, 2021, the employee went to sleep at his residence and the next morning on Sunday, April 25, 2021 the employee's daughter and wife found him unresponsive and dead in his bed.
