Investigation Summary
At 1:30 p.m. on August 16, 2021, an employee was raising the jacks on a trailer. The brakes had not been engaged on either the tractor or the trailer so when the jacks were raised they both rolled. The employee was crushed between the rolling trailer and the stationary trailer next to it. The employee died from his injuries.
