Investigation Summary
An employee worked as a Foreman overseeing Electricians. On July 16, 2021, the employee reported feeling ill. The employee was tested on July 19, 2021, and confirmed positive on July 21, 2021. The employee was hospitalized from July 23, 2021, to July 28, 2021. The employee informed Coworker #7 (Controller) on July 26, 2021, that he was in the hospital with pneumonia due to COVID-19. He thinks he contracted COVID-19 from Coworker #2 when they spoke to each other while not wearing masks on July 13, 2021, or July 14, 2021. On July 13, 2021, a Coworker #1 (Foreman Supervisor) had reported COVID-19 related symptoms. Coworker #1's spouse tested positive for COVID-19 on July 14, 2021. Coworker #2 had symptoms on July 15, 2021, and was tested positive that same day. He didn't notify his employer until July 19, 2021, when the Coworker #7 called him. Coworker #2 had symptoms on July 18, 2021, and was tested and confirmed positive on July 20, 2021. He said a Coworker #3 (Supervisor) brought pizza to work and he ate with everyone before he became ill. He does not know anyone outside of work that had COVID-19. Coworker #4 had symptoms on July 19, 2021, the same day he was informed by Coworker #2 that he had COVID-19. He was tested and confirmed positive on July 22, 2021. He though the pizza lunch was the source of his infection since they ate close together less than 6 feet apart next to Coworker #2's desk. Coworker #5 had no symptoms and was informed on July 20, 2021, to get tested. He was confirmed positive on July 21, 2021. He stated that he carpooled with Coworker #2 (his Supervisor). Coworker #6 had symptoms on July 20, 2021, tested on July 21, 2021, and was confirmed positive on July 22, 2021. He stated that he did not wear face cover because he was vaccinated. He also ate pizza with his coworkers. The employer did not have an effective COVID-19 Prevention Program in place.
