Event: 08/09/2021
Two Employees with COVID-19 infection are hospitalized, one

At 6:30 p.m. on August 9, 2021, Employee #1 who worked as a Warehouse/Ramp Agent at an international supply warehouse left work early on July 24, 2021, and did not report to work his next scheduled work day of July 25, 2021. According to the employer's medical staff, Employee #1 reported experiencing symptoms as of July 24, 2021. Employee #1 was not aware of any contact with ill persons, either at work or away from work. Employee #1 went for a COVID-19 test on July 25, 2021, at the hospital. The employee was hospitalized that same day at 07:25 p.m. and received a positive test result. Employee #1 was treated and discharged on July 26, 2021, at 1:52 p.m.. On July 30, 2021, Employee #1 was readmitted to the hospital at 11:16 p.m. and died on August 9, 2021, at 6:30 p.m. while still in the hospital. Employee #2 who also worked as a Warehouse/Ramp Agent last worked on July 15, 2021, and called off work on the next scheduled work day which was Jly 16, 2021. Employee #2 used a home antigen test for COVID-19 on July 22, 2021, and tested positive. Employee #2 was hospitalized on July 27, 2021, received medical care and was discharged on July 29, 2021. Employee #2 was released to return to work on August 6, 2021. In addition to Employee #1 and Employee #2, two coworkers had also tested positive for COVID-19, but only the two described in the narrative were hospitalized. The employer did not have an effective COVID-19 Prevention Program in place.
