Investigation Summary

An employee was working in an undetermined line of work. The NAICS code given, 333249, was for a manufacturer of industrial machinery. Elsewhere, it was stated that the employee worked at general medical or surgical hospital. Her occupation was not given. On July 22, 2021, she tested positive for COVID-19. It was possible that she was exposed to another employee who was COVID-19-positive, but also she had gone out of state for a time. The employee became sick while she was on vacation out of state. When she came home, she went to the same hospital where, by one account, she was employed. She died. Her employer was informed of the fatality at 6:57 a.m. on August 2, 2021.
