Investigation Summary
An employee was working as a telephone operator for a general medical and surgical hospital. She worked at a corporate facility. She and her coworkers worked in a room that was approximately 16 by 32 feet (4.9 by 7.3 meters) in size. Employees worked in cubicles with sides and did not work face to face. The employee began experiencing symptoms of illness on July 22, 2021. She received a positive COVID-19 test on July 25, 2021. She died on August 2, 2021, of respiratory issues due to COVID-19.
