Investigation Summary
An employee was working as a telephone operator for a general medical and surgical hospital. She worked at a corporate facility. She and her coworkers worked in a room that was approximately 16 by 32 feet (4.9 by 7.3 meters) in size. Employees worked in cubicles with sides and did not work face to face. The employee reported experiencing symptoms of COVID-19 on approximately July 16, 2021. She was hospitalized on July 22, 2021. She died on August 1, 2021, from respiratory issues due to COVID-19.
