Investigation Summary
At 8:00 a.m. on July 7, 2021, an employee was crouching on the ground using a Hougen Mag Drill to drill holes into galvanized metal embeds strapped to a pallet. The employee wore a glove on his left hand while operating the drill. During the process of drilling the 48th hole, the employee lost balance and fell toward the rotating drill bit. The glove and the employee's left thumb became caught and wrapped around the drill bit. The employee attempted to shut off the drill but was unable to reach the power button. A coworker saw the incident and disconnected power by pulling the drill's power cable, then assisted the employee in freeing his hand. The employee sustained a full amputation of the left thumb. The employee was hospitalized for treatment. The injury was reported to Cal/OSHA Long Beach District Office the same day, and an investigation was initiated.
