Investigation Summary
At 2:30 p.m. on July 27, 2021, Employee #1 was riding on the side of a forklift while another operator drove the forklift. A manager operating a different forklift in reverse struck the employee's right leg with the rear bumper while hauling a palletized load. The employee sustained bruising and a laceration to the right leg and foot from the direct impact. The employee was treated without hospitalization.
