Investigation Summary
At 6:45 a.m. on June 14, 2021, Employee #1 was attempting to adjust a Viking Turbo 504 (Serial No. 930034) while the machine was in operation. The employee placed their right hand in an area with moving equipment and the right index finger was pinched and amputated against a stationary part of the equipment. Employee #1 was transported by ambulance to Riverside Medical Center and underwent surgery for the amputation of the right index finger up to the second knuckle. Hospitalization was not required.
