FALL FROM ROOF

ON AUGUST 16, 1984, AT APPROXIMATELY 1125 AM, EMPLOYEE #1 AND FOUR OTHER EMPLOYEES OF KOETTER CONSTRUCTION CO. WERE WORKING ON THE ROOF OF THE HARSHFIELD BROTHERS INC. BUILDING AT 875 CENTRAL COURT,NEW ALBANY, INDIANA 47150. THEY WERE LAYING 4" THICK AND 6' WIDE VINYL COVERED INSULATION OVER THE ROOF STRUCTURAL FRAME AND THEN LAYING 26 GAUGE, 36" WIDE AND 15' TO 25' LONG, STEEL ROOF SECTIONS OVER THIS INSULATION, THEN CAULKING AND ANCHORING THE STEEL ROOF SECTIONS TO THE ROOF FRAME. EMPLOYEE #1 AND ANOTHER EMPLOYEE CARRIED A METAL ROOF SECTION TO THE PLACE WHERE IT WAS TO BE USED. EMPLOYEE #1 WAS ON THE HIGHER PART OF THE ROOF. HE STEPPED FROM THE METAL ROOF SECTION ONTO THE VINYL COVERED INSULATION AND FELL APPROXIMATELY 32' TO A SMOOTH ROCK SURFACE BELOW. HE WAS KILLED.
