Investigation Summary
At 11:10 p.m. on May 24, 2021, Employee #1 was attempting to remove a mold from a transfer pot using a standard pry bar. While putting his full weight on the bar, it slipped, causing him to fall and strike his legs against the lower crossbar frame of the steel table holding the mold. Employee #1 sustained a fracture to the left leg in two places and was hospitalized for treatment.
