FALL FROM STEEL SUPPORT BEAM

EMPLOYEE #1 WAS WORKING AS A RIGGER, PREPARING FOR THE "KISS" CONCERT. SOMEONE SMELLED SMOKE SO EMPLOYEE #1 TOOK A PORTABLE FIRE EXTINGUISHER UP TO THE CEILING AREA TO CHECK FOR FIRE. HE WAS WALKING ON A STEEL BEAM 47 FEET, 9 INCHES ABOVE THE FLOOR. NO FALL PROTECTION OF ANY KIND WAS IN USE. HE FELL TO THE CONCRETE FLOOR BELOW AND WAS KILLED.
