THREE EMPLOYEES WERE CHANGING 480-VOLT FUSES IN A DISCONNECT BOX LOCATED IN A STORAGE ROOM. ONE EMPLOYEE WAS USING CHANNEL LOCKS TO REMOVE A FUSE AND CAUSED A GROUND FAULT. THE RESULTING ARCS BURNED ALL THREE EMPLOYEES.
