Investigation Summary
An employee worked part-time in the print department of a wholesale packaging supplier. Prior to the employee being diagnosed with COVID-19, two coworkers were also diagnosed. The employee and six coworkers in the same department were diagnosed with COVID-19 in January of 2021. The employee was hospitalized and subsequently died due to complications from the infection at 5:00 p.m. on February 21, 2021. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
