Investigation Summary
At 7:50 a.m. on April 27, 2021, Employee #1 was operating a Jouanel swaging machine (model: SB7RVV) to bead a 12-inch diameter coupling ring with a 4-inch wide sheet metal while wearing Maxiflex Ultimate 34-874 gloves. The coupling ring snagged the employee's gloved hand, pulling it into the machine and trapping her left ring finger between the two dies. The employee sustained a partial amputation of the left ring finger and was treated without hospitalization.
