EMPLOYEE KILLED WHEN STRUCK IN ABDOMEN BY FLYING STEEL ROD

EMPLOYEE #1 HAD "CHUCKED-UP" A HALF INCH DIAMETER, 16 INCH LONG STEEL ROD IN A MILLING MACHINE/DRILL PRESS. HE SPEEDED UP THE MACHINE TO 3400 RPM TO POLISH THE ROD WITH EMERY CLOTH. CENTRIFUGAL FORCE ON THE ROD CAUSED IT TO BREAK AND FLY OUT OF THE MACHINE. A PART OF THE ROD STRUCK EMPLOYEE #1 IN THE ABDOMEN, LACERATING HIS AORTA. THE EMPLOYEE DIED OF THE INJURY. THE COMPANY DID NOT HAVE ANY WRITTEN PROCEDURES OR SAFETY RULES FOR THE OPERATION OF THIS MACHINE.
