Investigation Summary

Investigation Nr: 135299.015
Event: 05/02/2021
Employee with COVID-19 infection dies

Employee #1 was working for a metal service center and wholesaler in the firm's drill shop with four coworkers. The employees mostly worked separately on different machines, but they were required to share a crane and a press machine. The employer provided cleaning solution to clean shared equipment and emphasized its use. The employer also required face masks and social distancing, and encouraged employees not to come in to work sick. On April 1, 2021, while working in the drill shop, Coworker #1 started to experience the symptoms of COVID-19 and left work early. On April 3, 2021, Coworker #1 tested positive for COVID-19. On Saturday, April 3, 2021, Coworker #2 started experiencing symptoms and he tested positive for COVID-19 that same day. The employer was told about the two employees' test results on April 4, 2021. The employer then told the other two drill shop employees, who had come in to work, to leave the facility and get tested. Coworker #3 tested negative but Employee #1, tested positive. Employee #1 was hospitalized and died at 8:00 a.m. on May 2, 2021.
