Investigation Summary
At 9:15 a.m. on March 25, 2021, Employee #1 was attempting to pull netting through the accumulator of a machine to restart the process. As he pulled the material, the string wrapped around his hand became tighter, raising his hand toward the machine. In an attempt to free himself, he pulled downward, resulting in the amputation of his pinky and ring finger on his left hand. The employee was treated without hospitalization.
