EMPLOYEE KILLED BY GANG SAW NOT LOCKED OUT

AT 1:45 PM ON MARCH 17, 1987, EMPLOYEE #1, THE OFF-BEARER ON THE GREEN END OF A FRICK 48 INCH COMBINATION GANG SAW RAISED THE LID OVER THE BLADES WITH HIS LEFT HAND TO REMOVE A PIECE OF LOG CAUGHT IN THE SAW. ANOTHER WORKER MISTAKENLY STARTED, THEN IMMEDIATELY STOPPED, THE SAW. EMPLOYEE # 1 WAS DRAWN INTO THE MACHINE. HIS RIGHT ARM WAS PARTIALLY SEVERED. FOUR OF EIGHT 48 INCH SAW BLADES PIERCED THE RIGHT SIDE OF HIS BACK. THE BLADES WERE BACKED UP TO REMOVE EMPLOYEE #1, WHO HAD DIED. THE SWITCH FOR THE ELECTRICAL EQUIPMENT WAS NOT OPEN AND TAGGED, BLOCKED OR LOCKED OUT BEFORE WORK ON THE SAW WAS PERFORMED.
