Investigation Summary

Investigation Nr: 135170.015
Event: 04/26/2021
Employee with COVID-19 infection dies

An employee was working at the food counter of a supermarket. His last day of work was March 27, 2021. Up to the last day of his work shift, he appeared well and healthy to his coworkers. Sometime between March 27 and March 30, 2021, he began exhibiting symptoms of COVID-19. The employee's direct supervisor stated that the employee had texted him, saying that he was not feeling well, as if he were getting a cold, and that most likely he would not be in to work on March 30, 2021, his next scheduled work day. The employee drove himself to the hospital on his next scheduled work day and notified the nursing service for his employer to inform the employer that he was at the hospital. As per company protocol, the nursing service had the employee tested for COVID-19. He tested positive. At 8:00 a.m. on April 26, 2021 he died, due to the COVID-19 virus.
