Investigation Summary
At 2:53 a.m. on April 13, 2021, Employee #1 was retrieving labels in the work area when a temporary employee operating a forklift was moving forward with the forks raised approximately three-quarters high. The forklift was carrying a large, empty aluminum bin used for shipping interior auto headliners. As the operator made a left turn while raising the forks, the bin shifted forward and slid off, striking Employee #1, who was standing nearby. Employee #1 sustained multiple bodily injuries, including a cracked pelvis and a right knee fracture. The employee was hospitalized for treatment.
