CRUSHED BETWEEN MATERIAL AND RAIL

TWO EMPLOYEES WERE UNLOADING WIND WALLS WITH A FORKLIFT. THEY WERE USING THE FORKLIFT TO STABILIZE THE MATERIAL. EMPLOYEE #1 WOULD ATTACH A SHACKLE AND CHAIN AND THEN REMOVE HIMSELF FROM THE DANGER ZONE. THE WIND WALL WOULD THEN BE REMOVED FROM THE CARRIER. THEY WERE CALLED AWAY FROM THAT ASSIGNMENT TO DO ANOTHER. WHEN THEY FINISHED EMPLOYEE #1 RETURNED. THE OTHER EMPLOYEE WENT TO CHECK METERS. EMPLOYEE #1 WAS FOUND CRUSHED BETWEEN THE WIND WALLS AND THE RAIL. IT IS ASSUMED THE EMPLOYEE WAS ATTEMPTING TO PERFORM SOME ACTION WITHOUT USING THE THE FORK LIFT TO MAINTAIN THE STABIILITY OF THE LOAD. HE HAD BEEN CAUTIONED NOT TO DO THAT.
