Investigation Summary
At 7:30 a.m. on March 25, 2021, Employee #1 was pulling PH6 tubing out of a hole and standing it back in the derrick. The slip-type elevator presented a crushing hazard when positioned too far down on the tubing or when pinching the tubing at an angle. The company was aware of this condition due to prior experience with lifting tubing off flat surfaces and angled lifts on the platform. During the operation, Employee #1 sustained an amputation of two toes at the top of his toenails on the left foot. The employee was hospitalized.
