Investigation Summary
At 6:28 p.m. on March 26, 2021, Employee #1, employed by a printing company, was loading a shirt into the machine pallet while standing at the point of operation between the pallet and the print screen of the M&R Challenger III screen print machine. The machine rotated, pinning the employee between the pallet and the print screen. Emergency services were called, and the employee was transported to the hospital, where she was admitted and treated for a back fracture.
