Investigation Summary
At 11:30 a.m. on April 6, 2021, a coworker loaded welded steel pipe onto a Toyot a Forklift. The coworker used the forks to hoist the bulky and unsecured load un til the pipe was an inch or more off the ground in order to weld angle iron onto the pipe ends. He had asked an employee to stand by and be available in the eve nt that he needed something (materials, tools, equipment). The pipe then fell, s triking the employee in the head and killing him.
