Employee with COVID-19 infection dies

An employee was working as a machine operator for a firm that manufactured plastic products. He worked on the shop floor. At the facility at which he worked, there was an increase in COVID-19 cases in the month of March 2021. The first reported case of COVID-19 in the facility was on March 2, 2021. Over the next few weeks, 26 workers there came down with and tested positive for COVID-19. Because of the surge of cases, management decided to shut down for the weekend on March 19 in order for the facility to be sanitized as a deep cleaning was performed. On March 20, 2021, the employee tested positive for COVID-19. He went home to recover. On April 3, 2021, he was transported to a hospital. He was put on a ventilator, and he died on that same day, April 3, 2021. The employer was notified on Monday, April 5, 2021, of the death. On April 6, 2021, the employer reported the fatality. The employer did not report the work-related fatality to OSHA within 8 hours.
