Investigation Summary
At 6:00 a.m. on March 17, 2021, Employee #1 was working as a kitchen helper at a grocery store and was operating a Hobart mixer. The employee was preparing to use the Hobart to cut carrots when they noticed leftover cabbage bits in the pelican head attachment. The employee turned off the Hobart to remove the debris with their hand but became distracted and looked away. At the same time, the employee placed his hand into the pelican head, where the finger was cut by the still-rotating blade. There was one other employee present in the back kitchen, but the employee was facing away from Employee #1. The supervisor was in the front kitchen at the time of the accident. The employee suffered a laceration and open fracture to the left-hand ring finger, requiring hospitalization for three nights. Cal/OSHA identified two contributing factors: failure to clean the pelican head attachment after prior use, and the employee's distraction while handling the machine.
