Investigation Summary
At 12:30 p.m. on March 26, 2021, Employee #1 was removing the center steel bands securing four steel coils that were standing on end on a wooden pallet. The coils were not centered on the pallet, and when the banding was cut, a 3,000-pound coil at the end of the pallet tipped over, striking Employee #1's left foot. As a result, Employee #1 sustained a head laceration, a broken foot, and a broken ankle. The foot was later amputated at the ankle. Employee #1 was hospitalized.
