EMPLOYEE CRUSHED TO DEATH BY SCREW CONVEYOR ABOARD A SHIP

THIS ACCIDENT TOOK PLACE ABOARD A GRAIN-CARRYING SHIP EQUIPPED WITH WING TANKS ON EACH SIDE OF THE SHIP. A SCREW CONVEYOR RUNS THROUGH A LONGITUDINAL TROUGH IN EACH WING TANK. TWO WING TANKS WERE BEING WASHED. SIMULTANEOUSLY, A MARINE CHEMIST AND A SHIP-REPAIR COMPANY EMPLOYEE WERE INSIDE ONE OF THE OTHER WING TANKS NOT BEING WASHED. EMPLOYEE #1 WAS STANDING THE CONVEYOR TROUGH WHEN ONE OF THE SHIP'S CREWMEN TURNED THE CONVEYOR ON. HE WAS CAUGHT AND CRUSHED TO DEATH BY THE AUGER. THE SHIP'S CREW APPARENTLY WAS UNAWARE OF THE THE TWO MEN'S PRESENCE IN THE WING TANK. A LOCKOUT PROCEDURE WAS IN EFFECT FOR THE WORKERS WASHING OUT THE OTHER TANKS.
