Investigation Summary
At 12:15 a.m. on February 27, 2021, Employee #1, a sanitation worker, was preparing to clean a DSI model #044 portioning machine, serial #4320, as part of daily sanitation duties. While retrieving chicken in the DSI area, a high-pressure water jet stream activated unexpectedly. The water pressure, measured at 480 psi, caused lacerations to the employee's right middle and ring fingertips. The employee was hospitalized for treatment.
