Investigation Summary
At 8:00 p.m. on December 11, 2020, an employee who had been working as a Recepti onist was hospitalized due to COVID-19. The employee worked at a licensed skille d nursing facility that employs 80 staff including Registered Nurses, Certified Nursing Assistants, Receptionists, Housekeepers and others. In late November and all through December of 2020 there was an outbreak of COVID-19 at the facility. From November 28 to December 28, 21 coworkers tested positive. On November 28, 2020 "Patient 0", an asymptomatic dialysis patient, took a COVID-19 test which w as positive. Additional testing was conducted, and on December 7, 2020, two cowo rkers tested positive for COVID-19, triggering outbreak requirements including s egregation of patients into "green," "yellow," and "red" zones, for non-COVID-19 patients, patients who did not have a recent negative test, and COVID-19 cases, respectively. During this time the employee was tasked with screening employees for signs and symptoms of COVID-19. The employee performed this work wearing a fit-tested BYD N95 Respirator. While working on December 7, 2020, she took a COV ID-19 test which was negative. She continued to work to the end of her shift on December 9, 2020, and after returning home began to feel feverish. She called in sick the following day, and was hospitalized on December 11, 2020, when she tes ted positive for COVID-19. The employee stated that she did not enter patient ca re areas, took her lunch in an isolated office or in the break room, but only if no one else was present. Through an exposure evaluation the employer identified 3 coworkers who may have had close contact with the employee during her high-ri sk exposure period. One employee ate lunch in the same room but stayed at least 6 feet apart at a different table. The other 2 potential close contacts were rul ed out due to the use of respiratory protection at the time of contact. Adequate Aerosol Transmissible Diseases policies and procedures were not in place.
