Investigation Summary

Investigation Nr: 133636.015
Event: 01/02/2021
Employee with COVID-19 infection dies

An employee was a Warehouse Clerk and engaged in inventory, packaging, shipping, and receiving tasks. The employer engages in warehousing, packaging, and shipping of machinery parts. The worksite is divided into a warehouse with attached office. On December 9, 2020, the employee worked his last day before exhibiting COVID-19 related symptoms and being sent home for testing and quarantine. The employee tested positive for COVID-19 on December 11, 2020. On December 18, 2020, the employer received the results and conducted contact tracing, testing any potential close contacts. One coworker who had tested negative for COVID-19 on December 18, 2020, was tested again on December 21, 2020, and tested positive. The employer closed business operations for the remainder of 2020 and sent all employee's home. Most of the employee's coworkers had already been working remotely since the start of the pandemic. The employee died at home at 2:00 p.m. on January 2, 2021, after having not sought medical treatment for COVID-19. The coworker who tested positive did not incur any COVID-19 symptoms and had an antibody test conducted for COVID-19 on February 16, 2021, which was negative. The conclusion was that the coworker never had COVID-19 and had a false positive. It is not known how the employee contracted COVID-19 as none of his coworkers tested positive for the illness.
