Investigation Summary
An employee a Lead Cook, last worked from 6:48 am to 10:00 am. Approximately two hours into the shift the employee started to not feel well. The employee was sent home by supervision and the employee had a COVID-19 test administered by the employer's contracted testing service at the establishment prior to leaving. On or about December 11, 2020, the employee received a positive COVID-19 test result. On December 15, 2020, the employee's symptoms increased to include a sore throat, fever, chills, and mild to severe shortness of breath. Due to the increased symptoms, the IW had a telemedicine appointment with their physician who advised the employee to go to the emergency department due to shortness of breath and issued prescriptions. On or about December 17, 2020, the employee was hospitalized due to hypoxia-related to COVID-19. At 9:00 a.m. on February 13, 2021, the employee died.
