Investigation Summary
On December 15, 2020, three employees who worked in an office environment with cubicles or in a warehouse packing items into boxes were reported hospitalized with COVID-19. The employees were hospitalized between November 2020 and January 2021. The employer had policies to protect workers from contracting COVID-19 including mandatory mask wearing, social distancing rules, barriers, daily temperature and symptom screening, frequent disinfection of surfaces, rules for return to work following illness and contact tracing notifications for employees potentially exposed to an infected coworker. One employee died from his illness on January 21, 2021, due to related health issues and severe acute respiratory disease from COVID-19.
