Investigation Summary

Investigation Nr: 134203.015
Event: 12/19/2020
Employee with COVID-19 infection dies

Employee #1 was working on November 19, 2020, when the employer received a call from a coworker claiming she was sick. The employer sent the coworker for a COVID-19 test and she tested positive the same day. The employer then sent three potentially exposed employees, including Employee #1 for COVID-19 testing on November 20, 2020, and all three tested positive. The employees all worked in different areas of the operation, two in the warehouse but spaced, and two in production but spaced. The employer also indicated that these four were the only positive tests in the company as, after these four tested positive, the company tested all remaining on-site employees and their shared-home family members. Due to the employees all testing positive, the employer shut down the facility for five days for a deep cleaning. Employee #1 was hospitalized on November 27, 2020 and died on December 19, 2020.
