HIT ON HEAD BY FLYING CHIPPER HOOD.

Employee #1 was standing at the conveyor table taking lumber off the conveyor table. A blade came off an operating chipper located 39 feet away from Employee #1. The blade knocked the chipper hood off. The hood struck employee #1 in the head, killing him.
