Investigation Summary
On December 5, 2020, an employee who worked as a Registered Nurse for a skilled nursing facility died from COVID-19. The employee was at home ill when she died. The employee is believed to have contracted COVID-19 at work. The employee worked directly with residents that were suspect or confirmed positive for COVID-19 and had worked during an outbreak at the facility. The employer did not have an adequate Respiratory Protection Program in place.
