Investigation Summary
At 12:45 p.m. on January 20, 2021, an employee was reaching into the feeder wheel on the packaging line (before the horizontal bagger) to pull out a fortune cookie. The employee's right ring fingertip got caught in the feeder wheel. The employee suffered a fingertip amputation and was treated without hospitalization.
