Employee with COVID-19 infection dies

At 3:15 p.m. on January 23, 2021, an employee who had been working as a Physician at a medical office died from COVID-19. On December 31, 2020, the employee received a positive COVID-19 test result just prior to going on vacation. The employee notified management of the positive test result and management notified the coworkers who worked closely with the employee, including his secretary and medical assistant. Between January 1, 2021, and January 4, 2021, Management notified the remaining coworkers who were all clinic personnel. Between December 31, 2020, and January 5, 2021, a total of 12 out of 22 coworkers tested positive for COVID-19. The employer had not notified employees within 24-hours of their contact with a COVID-19 positive employee as required. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
