Investigation Summary
In late 2020, an employee was working as a licensed practical nurse (LPN) at an assisted living facility for the elderly. She worked as a full-time night shift nurse at the facility's station 1. She did not work on station 2, which was the COVID-19 unit. She was a charge nurse. She oversaw certified nursing assistants {CNAs) and made passes each night from 1:00 p.m. to 7:00 a.m., Monday through Friday. She cared for approximately 25 residents. The director of nursing revealed that there had been a COVID-19 outbreak at the facility. It had begun on December 29, 2020. The employee had been in contact with other employees who had tested positive. She had also been in contact with three of the infected residents who had experienced no symptoms. On the employee's last day of work, January 1, 2021, she did not indicate on the daily COVID-19 screening form that she was experiencing any symptoms of COVID-19. Three days later, on January 4, the employee arrived at the center and was tested in her vehicle by the director of nursing. The employee reported to her work facility for a COVID-19 test but remained in the car with her brother. She was experiencing symptoms of COVID-19, such as coughing, at the time of testing. The center used a rapid antigen test, which gave results within 15 minutes. The employee tested positive for COVID-19. The director of nursing encouraged the employee to see her primary care physician and to isolate for at least ten days. At 4:30 p.m. on January 13, 2021, the employee died.
