Investigation Summary
At 12:30 p.m. on December 29, 2020, Employee #1 was assisting a forklift operator in moving a large piece of granite to an outside dumpster using a forklift equipped with a crane attachment. While walking alongside the granite to help steady it during transportation, Employee #1 tripped and fell due to a hole in the asphalt. The forklift operator, unaware of the fall, ran over Employee #1's left leg with the forklift tire. Employee #1 sustained a broken left ankle and three broken toes on the left foot. The employee was hospitalized for treatment.
