At 8:00 a.m. on January 11, 2021, an employee worked as a Shift Manager at a fast food restaurant. The employee took three days of vacation and became ill with fever on the third day. He tested for COVID-19 on 1/4/21 with positive result on 1/6/21. He died at home on 1/8/21 at 5:00 PM.
