Investigation Summary
At 10:45 a.m. on January 6, 2021, Employee #1 responded to a request for assistance from a coworker, who was attempting to rewind a steel coil. Employee #1 had been training the coworker on press operations. While the coworker was working the controls to rewind the coil, the steel began "bunching up" because all of the hold-downs, which maintain tension on the coil, were released. Without sufficient tension, the rewind occurred faster than normal, causing the coil to become crooked. Employee #1 began assisting the coworker in straightening the coil at the mandrel but failed to lock out the equipment. While positioned between the coil and the coil blade, Employee #1 manually adjusted the coil. As Employee #1 was holding the loose coil winds in place, he instructed the coworker to operate the coil hold-down arm. However, the coworker mistakenly activated the "coil blade" button instead of the "coil holder" button. The coil blade moved toward the coil, pinning Employee #1 between the coil and the blade. Employee #1 was trapped for approximately five minutes and, upon being released, required CPR. Employee #1 was hospitalized to treat bruising and crushing injuries to the upper chest, lower neck, and side of the face, along with a collapsed lung.
